Terms and Conditions of Assess and Educate

 

1.       Fees 

I tailor what is required according to the needs of each client, and therefore I will provide a quote in writing, based on your individual requirements. Written acceptance of the fees will be required prior to the commencement of the work.

2.       Payment terms 

Upon booking an individual consultation, an invoice will be sent to you. Invoices should be paid on the day of receipt. Failure to settle invoices may result in appointments being delayed or cancelled. Please ensure that the email address provided on the personal information form is the email address that you would like your invoices sent to.

3.       Cancellations

As much notice as possible should be provided if you are unable to attend a consultation. If you miss your appointment or if you cancel within 24 hours of your session, I will need to charge the full fee for that time although I will endeavour to reschedule at the earliest opportunity. Please advise if you need to miss or cancel an appointments via email to assessandeducate@gmail.com

4.       Confidentiality 

I confirm that our sessions are confidential except as required by law or as provided for in regulatory, ethical or other professional statements relevant to my engagement.

Where possible, I will always inform you first and discuss with you, if confidentiality is going to be broken.

5.       Record keeping and data protection  

Your data is processed in line with the General Data Protection Regulation 2018 and professional practice guidelines. Your personally identifiable data is only processed by myself and any other parties to whom you have given written data-sharing consent.  

When making notes in session, I do not include any identifying information (e.g., name, initials, or time/date of session).

Any electronic client records, and/or the devices on which they are kept, are encrypted and/or password protected. I do not transport paper material with identifying information. I take steps to ensure that no one else can access your personal data without your specific consent. 

In the unlikely event of a data breach, I will report this to you and to the Information Commissioner’s Office within 72 hours. You may request copies of the data I hold about you. I will respond within 30 calendar days. There is no fee for this. 

I will retain your data for seven years, in line with professional practice guidelines. At that point they will be destroyed/erased, to include all electronic communications. 

In certain circumstances, you may wish to have your personal data permanently erased sooner than this. This is known as the right to be forgotten. To allow me to do so, I will need a written request from you via email to (insert new email)

6.     Your Rights 

You have the following rights:
•    Right of access: the right to make a written request for details of your personal information and a copy of that personal information
•    Right to rectification: the right to have inaccurate information about you corrected or removed

After initial contact a date will be agreed to conduct the assessment. You will be sent a confirmation email with these terms and conditions and details to make a deposit to secure the session.

The date will only be secured once the deposit is paid.

You will receive a background questionnaire and a school questionnaire for the SENCO to complete. This will need to be returned minimum three full weeks prior to the assessment, along with any other professional reports. (eg. OT, EHCP etc)

It is important that vision and any visual difficulties have been checked prior to an assessment or this could cause delays to the assessment taking place. Please ensure an eye test has been carried out by an optometrist (and, as appropriate, by an orthoptist) within 6 months of the assessment date.

Face to face Assessment

I work in and around NW London. If an assessment is requested further, an additional cost for travel and time will be applicable. Travel time is charged at the hourly rate of SEND Consultancy (currently £150, as at February 20th 2024). It is best if the assessment takes place in a setting familiar to the learner to ensure they are at ease. This usually takes place at the learners' school, if the school gives permission and they have space available. This will be discussed during the initial consultation and it is the parent/carer that is responsible to provide contact details for the learners educational setting as well as making an initial request to conduct the assessment at the learners school. This must be confirmed within a week of our initial discussion.

Secure the assessment date/time

In order to secure a booking, a deposit of £200 is requested. This will be used to secure the date and to cover expenditures. This deposit is non-refundable in the event of cancellation, including illness. An invoice will be provided, including the deposit and the balance of the assessment fee. The balance should be paid no later than one week prior to assessment. Failure to do so could result in loss of deposit and cancellation of the assessment.

Cancellation policy Cancellation by the client:

All changes and cancellations must be notified in writing to assessandeducate@gmail.com Cancellation charges will be applied as follows:

Cancellation/reschedule within 30 days of a booked session = 50% of fee. Deposit non refundable

Cancellation/reschedule within 48 hours of a booked session = 100% of fee.Deposit non refundable

Cancellation by Assessor

In the event that severe weather or any other emergency results in the cancellation of a session, Assess and Educate will endeavour to give clients a minimum of 24 hours’ notice and to reschedule the assessment. Please ensure you have provided up to date contact details (email and phone number) that are checked regularly.

After the face to face assessment

Please allow 3-4 weeks for a full written report and to discuss any questions or concerns that may arise.

Occasionally an assessment profile may not be indicative of a Specific Learning Difficulty. Often, in such cases, other difficulties may appear to be compounding the challenges identified in assessment. In these instances, a full report is provided (without diagnosis) and will still include recommendations to support areas that have been identified as relative weakness in the learner’s profile of strengths and challenges. Referrals to other professionals such as Psychologists, Speech and Language Therapists, Orthoptists, Behavioural Optometrists, Psychiatrists, Occupational Therapists, etc. may be included if there are indicators present of other learning differences that I am not qualified to diagnose such as ADHD, ASD, Visual Difficulties, to name a few.
